Certification Fee - $45 made out to "Treasurer, State of Ohio"
Clinic Fee - $20 cash to "Ohio Orthopaedics & Sports Medicine, Inc."

Effective June 9th, 2004, the State Board will
now issue a permit, instead of a validation,
to direct, supervise or coach a pupil activity program.

• Intended for individuals that direct, supervise or coach a physical activity program that involves athletics, routine/regular physical activity or health and safety considerations, must hold this permit.
• The permit is valid for up to 3 years and is renewable.
• All coaches (paid or volunteer) in all member schools (public and non-public) must meet the state standards and receive both the permit and have current CPR certification.
• Every coach is required to stay for this two and a half (2 1/2) hour course.
• We do not currently offer CPR certification.
